Patient access to electronic health records on health care
Digital health platforms have modernized and redefined the healthcare system and how patient receive their care. The PHR has allowed for patients to standardize and assume responsibility for how their health information is shared, how they engage with providers and understand their rights to safety and quality care. This blog will look at how PHR's not only diversify services offered but with provide further discussion of how providers can supporting patient decision making, manage chronic diseases, improve communication and optimize healthcare utilization.

Patient understanding of their health concerns
Access to online portals to review labs, reports, medications and treatments
Direct access to records allows for patients to have a better picture of the care they receive
Allowing this clarity give the patient a better ability to understand their conditions and make informed decisions based on their health literacy

Provide a well organized path for tasks involving medication refills, education and appointment scheduling
Minimizes risk for non compliance

Patient portals with secure messaging features allow for continuity of care
Establishes better patient rapport and collaboration between other interdisciplinarians

Tracking medications, log patient data and symptom monitoring
Real time monitoring of patient data
Gives the provider the ability to intervene earlier

Patients are more informed and more likely to explore their disease and treatment plan further
